This month we kick off 2021 with a focus on suicide risk and suicide prevention. NPSG 15 went into effect 2 years ago and despite tremendous efforts by healthcare organizations across the county, suicide rates have increased, and environmental risk continues to persist as a common survey finding. This month’s documents are designed to assist with staff preparation and increased patient safety.

Suicide Ligature Risk Assessment for the Environment of Care Tool – comprehensive ligature risk assessment tool to identify risks and document mitigation actions. This tool can be used as part of a cyclic ligature risk assessment.
Risk Assessment Template
Environmental Ligature Risk Assessment for Individual Patient Care Template – the template is used to assess the environment for self-harm risks during changes of 1:1 sitters or at shift change for at risk patients being monitored in non-behavioral health, individual patient care areas such as the Emergency Department, Medical/Surgical Units, and other non-behavioral health locations.
Immediate Threat Tracer Tool
Suicide Immediate Threat Tracer Tool – a tracer tool to be used by leadership for rapid rounding for common pitfalls with suicide risk compliance.
